We Accept Most Insurance Carriers

Please have your card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of visit. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service.


We participate with most insurance plans. Make sure we participate with your insurance plan. 


It is your responsibility to know the limits and coverage of your particular health insurance policy, to show your cards to us at each visit, and be prepared to pay any copays at the time of service.


Our billing staff will do their best to assist you with insurance questions; however, if you have questions about your coverage, it is best to check with your specific insurance company.


Our office does not want you to be surprised by a bill, but we must always bill your health plan based on federal guidelines and the actual services provided.


Please call our office if you need more information regarding insurance or billing.


Payments that are not covered by your insurance, such as copays and not covered services,  are expected at the time of your visit. If you need to make other payment arrangements, let us know when making your appointment or at the time of your visit. Payments can be made to our billing company, at the office, by mail, credit card, check or on your Portal.
